Qualchoice Health Plan
Qualchoice Health Plan provides a variety of group and individual health insurance plans and related services to members in northeastern Ohio.
Qualchoice offers PPO, HMO, point-of-service, and Medicare supplement plans, as well as vision and dental insurance coverage.
The Qualchoice provider network includes more than 4,700 physicians at 35 hospitals throughout their 11 county service area.
QualChoice has discontinued their Commercial and Medicare lines of business effective December 31, 2006. Services rendered while members were covered by QualChoice will continue to be handled by QualChoice staff.
